Required documentation
What insurance typically needs
Different items have different requirements. Here's what's usually needed for some of the most requested products — and remember, our team gathers all of it for you.
E0603 — Electric Breast Pump
Proof of pregnancy or recent birth and, for most plans, a prescription or order from your provider. Some plans also have timing rules for when the pump can ship.
A4287 — Milk Storage Bags
Typically dispensed alongside a covered pump. A prescription may be required depending on your plan and the quantity requested.
L0648/L0631, L0650/L0637 — LSO Back Brace
A prescription from your OB/GYN, midwife, spine physician, or other qualified provider, plus documentation of medical necessity for a maternity lower-spine or abdominal support brace. Common qualifying conditions include pregnancy-related low back pain, pelvic instability, lumbar strain, postpartum recovery, or diastasis recti.
L0625 — Abdominal / Back Brace
A prescription plus documentation of medical necessity, such as post-cesarean recovery, back pain, or pelvic instability.
A6530 — Compression Stockings
A prescription specifying the compression level (often 20–30 mmHg or higher) and a qualifying diagnosis such as edema or venous insufficiency.
A4670 — Blood Pressure Monitor
A prescription and documentation of a qualifying condition, such as gestational hypertension or preeclampsia monitoring.
A4467 — Maternity Belly Band
A prescription or written order plus documentation of medical necessity, such as pregnancy-related low back or pelvic girdle pain, round ligament pain, or abdominal support needs. Coverage for belts and bands billed under A4467 varies widely by plan, so we verify your specific benefits first.
L0621 — Postpartum Recovery Bodysuit
A prescription plus documentation of medical necessity for a sacroiliac or lower-torso support garment — commonly postpartum pelvic instability, sacroiliac joint pain, or recovery following delivery.
E0190 — Full Body Pregnancy Pillow
A written order and documentation of medical necessity for a positioning cushion, such as pregnancy-related back or pelvic pain, sciatica, or a condition requiring supported side-lying positioning. This item is plan-dependent and often reviewed case by case.
A7005/A7003 — Nebulizer Compression Kit
A prescription for inhalation medication plus a qualifying respiratory diagnosis (for example asthma or bronchospasm). Insurers distinguish between reusable administration sets (A7005) and disposable sets (A7003), and typically limit how often replacement supplies can be dispensed.
E0570 — Pediatric Nebulizer Compressor
A physician order for the compressor along with a covered inhalation drug and documentation of a qualifying respiratory diagnosis. Most plans require that the medication be one that must be administered by nebulizer, and the compressor is generally provided with an appropriately sized pediatric mask.
